During our trip next month we will be working with children at a YMCA’s early childhood care or day camp program. YMCA’s like these provide affordable childcare to low-income residents in many of Chicago’s most impoverished neighborhoods. Because the YMCA receives funding from the State of Illinois to help finance these programs, all volunteers need to comply with state regulations.
The state requires every volunteer, regardless of age, to sign a Facility User Agreement. While working at the YMCA, you may do activities with the children in the gym or pool (if there is one). It’s pretty standard for organizations with such facilities. Everyone in our group must complete this form and return it to me before May 27.
The state also requires that all volunteers under the age of 18 submit a proof of physical examination. This can be as simple as obtaining a photocopy of a student’s last school physical exam. If you are unable to get a copy of that physical, the student will need to have their doctor complete the YMCA Physical Record.
Please note that this record (either the photocopy or the YMCA document) must include proof of a negative TB test. However, you do not need to include vaccine records. Those younger than 18 must either complete YMCA Physical Record or obtain the necessary photocopy.
All volunteers 18 years or older will need to have a doctor complete the Adult Volunteer Medical Form. This is a simple physical exam, including a TB test. Everyone who is 18 or older must complete this form.
